Family Update

Directions:

1. Please view your family's entry in the 1985 blue St. Mary's Centennial book to see if the entry had any inaccuracies or turned out to be incomplete (for instance, birth and deaths occurring after the publish date). You can enter this additional information into the "Update/Correction" area of the form below, OR create a completely new and updated family entry. Either way the editors will use the best combination of your information to create an entry. If you do not have a copy of the 1985 book to look at, please request a scan of your family's entry, available only by email: rroder@midlands.net.

2. Fill out the form below as carefully, accurately, and completely as you can. Use one form for each household (can be a family that currently lives OR formerly lived in the parish).

3. Provide a photograph for your family (optional); you can drop a physical photograph at the parish rectory or upload (preferred) a photo at min. 360 dpi.

Please include:             

- Parents and birth/death dates and places for all family members.              
- Current city/state of residence for all family members.               
- Education backgrounds.            
- Occupations. 
- Military service.              
- Special accomplishments.              
- Churches, organizational memberships, etc.   
- Any ancestors you may wish to include, especially parents and grandparents.




Please be as inclusive with your available information as you can; your entry will be edited as necessary without losing any information.
